Internal NASA document signed by two moonwalkers.
Charles Conrad Jr. Signed (Twice) NASA Officer Leave Request Form, Approved and Signed by Alan Shepard, Originally from Conrad's Personal Collection. A partially-printed document titled "Officer Leave Request and Authorization" (form NAVPERS 2644), 8" x 5.25", filled in by typewriter and by hand, Houston, Texas, October 15, 1968. Conrad asks for two days of regular leave on October 17 and 18, signing twice: "C Conrad Jr". The request is recommended and granted by the Chief of the Astronaut Office: "Alan Shepard/ Capt USN". It is archivally mounted to a handsome document stating that it was "From the personal space collection of Astronaut Charles Conrad, Jr." Excellent condition.Auction Info
