Nobel Laureate Richard Feynman & the Space Shuttle Challenger (STS-51-L) Disaster: His "Quality Assurance and Reliability" Mock Exam, Directly from His Estate. Two stapled pages, 8.5" x 11". An eight question, multiple choice "Exam, Section 513" with the instructions: "Choose one answer for each question. If you do not know the answer leave question blank. Do not guess for credit will be subtracted for incorrect answers." This was apparently created by Feynman, a tongue-in-cheek critique of the political nature of the Rogers Commission. Sample questions below.3. The most important consideration for an investigative commission is:
a) relationship to the press
b) relationship to the management of the organization being investigated.
c) the subject being investigated
4. Advancement in a technological organization depends primarily upon ability to deceive:
a) Peers
b) Management
c) Nature
d) Funding organizations
6. The first order of business for a new commission is:
a) determine who is to write the final report
b) assigning office space
c) ensuring that none of the commissioners begin work
7. A good commission chairman should reserve for the final three meetings consideration of:
a) the content of the report
b) wordsmithing the report
c) the color of the report covers
Interestingly, it was announced on December 12, 1986, that "Kennedy Space Center will create a top division for safety, quality assurance and reliability as part of a major reorganization to streamline shuttle management and to improve communication at the launch center. The KSC changes involve new assignments for nearly every top shuttle official, most of whom were involved in the January 28 launch of Challenger."
It is unknown if Feynman distributed this exam to other Commission members. A fascinating document. Very fine.
