Apollo 13: NASA "MSC Apollo 13 Investigation Team Official Communication" Book. "Panel 6 - Related Systems Evaluation - Inputs from Reliability and Quality Assurance". A massive softcover book published at the Manned Spacecraft Center, dated May 15, 1970, NB/70-M-540. Approximately 488 screw-bound pages, 8.5" x 11". From the "Scope" of the publication: "One objective of this panel was directed toward related systems evaluation with emphasis toward oxygen/oxidizer type tanks and oxygen/oxidizer subsystem components using electrical type sources. Evaluation of the oxygen cryo tank is included under Panel 1 responsibilities, and is not covered in this report... The second objective of Panel 6 was to review potential changes to improve the Apollo spacecraft safe return. This second objective was not to be completed as a part of the Apollo 13 investigation but, was intended for identifying safe return improvements for future study."
