It is dated September 1, 1969, and signed in facsimile by the Apollo 13 original crew: "James Lovell", "Fred W. Haise", and "Ken Mattingly". It is signed next to the title: "To Neil, In Admiration of your years of 'Moon Lighting' in the Stock Market, Please Accept 1,000 Shares!/ Fred Haise Apollo 13 LMP".
