From the personal collection of longtime NASA photojournalist Andrew "Pat" Patnesky.

The photo description is found on the verso: "LLTV FLIGHT----Lunar Landing Training Vehicle No. 2, piloted by Astronaut Neil A. Armstrong, goes through a checkout flight at Ellington Air Force Base on June 16, 1969. Armstrong is the prime crew commander of the Apollo 11 lunar landing mission. During the lunar simulation flight he made two liftoffs and landings. The total flight time was six minutes and nine seconds; and maximum altitude attained was 300 feet.
