ID Plate from Snoopy, the First Lunar Module to Fly to the Moon.

Apollo 10 Lunar Module Flown Spacecraft Identification Plate Display Directly from the Personal Collection of Mission Command Module Pilot John Young, Certified and Signed. A metal plaque of 5.25" x 1.75" mounted to a 10" x 11" wooden display plaque beneath a metal die-cut representation of the lunar module. Engraved on the plaque from the Grumman Aircraft Engineering Corp. is the following:
APOLLO X LUNAR MODULE - 4
COL. T. STAFFORD CDR J. YOUNG CDR E. CERNAN
above the Part No. "LDW 280-54000-1, the Serial No. "001" and other information such as the Dsgn Cont No. "1st Lunar LM", Contr No. "NAS 9-1100", and "LAUNCH 5-18-69, SPLASHDOWN 5-26-69".

Three of these Lunar Module 4 Identification Plates were flown to the moon on Apollo 10 and returned to Grumman Aircraft Engineering Corp. who then mounted each ID plate on a shield-shaped wall plaque; one was presented to each crewmember. LM-1 flew on the unmanned test flight of Apollo 5. LM-2 never flew and is in the National Air and Space Museum. LM-3 flew in Earth orbit with Apollo 9 where it passed all its docking and extraction tests. LM-4 (Snoopy) flew with Apollo 10 to the moon where, in further tests in preparation for a moon landing, it separated from the Command Module Charlie Brown and descended to within 8.4 miles of the lunar surface. Extremely fine condition with just a couple of scrapes to the wood at the right edge.

At the bottom of this wooden plaque, Young has written in silver: "Plaque Flown on Apollo X. John Young".
