As follows.
Apollo 12 (November 14-24, 1969): Obtained from NASA's Terry N. Slezak of the MSC Lunar Receiving Lab.
Apollo 14 (January 31- February 9, 1971): Obtained from NASA's Terry N. Slezak of the MSC Lunar Receiving Lab.
Apollo 15 (July 26- August 7, 1971): Obtained from the collection of Navy Recovery Team Diver Alan Buehler.
Apollo 16 (April 16-27, 1972): Obtained from the estate of a North American Rockwell engineer who removed it during post-flight testing.
