A metal ID plate of 5.25" x 1.75" mounted to an 8" x 11" shield-shaped wooden display plaque beneath a 5.25" x 4.25" metal die-cut representation of the lunar module. Engraved on the plate from the Grumman Aircraft Engineering Corp. is the following:

Part No. "LDW 280-53000-1" / Serial No. "033"
Dsgn Cont No. "First Flight" / Contr No. "NAS 9-1100"

Mfd. By Grumman Aircraft Engineering Corp.
Bethpage, New York U S FS Code 26512

The front of the plaque has been signed by Schweickart in silver felt tip, as follows: "LM I.D. Plate Flown On/ Apollo 9/ Rusty/ Schweickart". Note that Schweickart flew as a civilian, thus no rank listed on the plate.

This Lunar Module-3 Identification Plate was flown on Apollo 9, the important first flight of the Command/ Service Module and the Lunar Module together, and returned to Grumman Aircraft Engineering Corp. They then mounted this ID plate on a wall plaque and presented it to Schweickart. Rarely offered and an extremely significant part of space history, worthy of inclusion in the finest collections. Very fine overall with only a few chips, dings, and glue residue on the wooden plaque.
