Confederate Generals Preston Smith, Benjamin Cheatham, and Leonidas Polk Endorsements Signed. The endorsement panel, which measures 3" x 7.75" and has been removed from a larger document, is docketed at the upper edge: "Report of board to examine and review testimony in cases of applicants for discharge under Conscript Act." The document is undated, but dates from 1861 until sometime before September 19, 1863, the death date of Gen. Preston Smith, who was killed during the Battle of Chickamauga when he accidentally rode into the Union lines during a night raid. Smith's death predated the death of Gen. Polk, who died on June 14, 1864, near Marietta, Georgia. Below the docket, each of the generals has placed their endorsement, in full:

Benjamin Cheatham (1820-1886), a veteran of the Mexican War and a miner during the 1849 California Gold Rush, served as a major general with the Confederate Army of Tennessee and saw action at the Battles of Shiloh, Perryville, Chickamauga, and others before being blamed for dereliction of duty leading to the escape of Union Gen. John Schofield at the Battle of Spring Hill.
