    Gordon Cooper and Deke Slayton: Signed 1967/68 Field Grade Officer Effectiveness Report for Charlie Duke, from his Personal Collection. This 8" x 10.5" pre-printed, two-sided form was filled out and signed on April 23, 1968 by "Leroy G Cooper Jr" (1927-2004), Astronaut, and "D K Slayton" (1924-1993), Director of Flight Crew Operations. Duke was given high praise indeed by his superiors with "Absolutely Superior" checked under Overall Evaluation, and "Outstanding Growth Potential" checked in Promotion Potential. The Comments area mentions his civic responsibilities: "...Major Duke actively participates in local civic organizations such as the Rotary Club and the Boy Scouts of America. He frequently speaks to chamber's of commerce, and secondary and college level student bodies. He participates in local church activities... He is an outstanding example of an officer in which the Air Force can have great pride." Extremely fine condition.

    Included with this lot is a handwritten and signed Letter of Authenticity from Charles M. Duke, Jr. on his personal letterhead stating, in part: "[this report] was a part of my official file as a USAF officer. [It] is an evaluation of my performance as an astronaut [and] was used to determine my eligibility for promotion... It is signed by Leroy (Gordo) Cooper, my immediate supervisor and Donald (Deke) Slayton, Director of Flight Crew Operations. They were both members of the 'original seven' group of astronauts... This report has been a part of my personal records since I retired from the Space Program. May it remind you of the dynamic days of Apollo."
