The plaque has an engraved plaque which reads These items flown on Apollo IX, March 3-13,1969. Presented to: Fred Haise. The back of the plaque is covered with green felt and I have inscribed it from my personal collection of NASA memorabilia. Apollo 9 was the first manned flight of the Lunar Module. The crew of Jim McDivitt, Dave Scott and Rusty Schweickart tested many of the critical aspects necessary to land man on the moon such as docking maneuvers, navigation systems, spacesuit/backpack abilities and the LM's engine. Astronauts were often assigned to support crew and backup crew duties before being assigned to the prime crew of a flight. I was a member of the Apollo 9 support crew along with Jack Lousma, Ed Mitchell and Al Worden and was given this plaque from the crew upon their return. This Bendix electrical connector has been in my personal space artifact collection since 1969 which is now 40 years after the flight of Apollo 9."
